I received a number of replies to my request below. Thank you to everyone for the enthusiastic response. To clarify, I wanted to make a few points.
I am not developing a national referral list for general use. That type of info is available via the IARP membership directory, AANLCP Find-a-Nurse, ICHCC web site, & the Care Planner Network directory.
I am making a list for my use to provide to my referral sources when I am unable to assist them due to my time constraints.
My list will include Name with credentials, phone number, email address + whether the person is new or experienced. In other words, I am providing contact info. Therefore if you emailed me & did not include your contact info IN your email, please provide that.

We in Pittsburgh have many blessings (Don't forget the IARP/ISLCP conference in October!). One of our blessings seems to be that there are more referrals for life care planing and vocational expert services than there are actual experts. I routinely provide referral sources with names and contact info of other experts when I am unable to take a case. For a variety of reasons, I really need to expand my referral list.
If you would be interested (and have the capacity!) in being included on the new list I am creating, please let me know via private email. Please note that I am requesting you let me know if you are experienced or new to expert work. In this manner, I can create 2 lists to provide to my referral sources as appropriate.
