  • 1.  Treatment for life time for non cat. cases

    All, I had a conversation with an attorney earlier and he asked me if there is any thing in our CLCP literature supporting lifetime treatment for  a patient who underwent lumbar and cervical surgeries at ate 41. The treating ortho recommends that  beyond a reasonable degree of medical certainty, his patient will need anti inflammatory, pain meds, anti muscle spasm meds for their lifetime -he included blood testing for monitoring blood levels. Any feedback appreciated as for articles supporting or refuting such.

    Just curious. What is his current status?  Is it already a 'failed back' situation at this time?  Is he currently requiring those medications post operatively?

    We have some literature, but need more details.  Does the person have cuada equina, stenosis, failed back, pseudoarthosis, successful fusion (or not), other structural damage, etc.?

    The front page of the March 16 issue of USA Today carried a story that the CDC has just come out urging doctors to avoid prescribing powerful opiate pain-killers for patients with chronic pain, saying the risks far outweigh the benefits for most people.

    Ted, I'm confused by your question. By definition, a life care plan is appropriate for one with "chronic health needs." We see life care plans for post fusion individuals all the time. If you have medical foundation for these future care needs, I believe that's all you need in order to write a life care plan consistent with those medical recommendations.  I would, however, get specificity from the doc as to frequency, duration of treatment and specific medication dosages, frequency of lab work, etc. that he is recommending as you could not do that as a non-– medical expert.  Why try to "bolster" a life care plan with "research" as you have this medical opinion?

    Can you expand a bit on the reasoning behind your question?  Why do you feel that you need "articles" when you have an orthopedic expert giving you general recommendations that you can clarify with a medical conference?  

    What I see too often are life care planners making only research – based recommendations for a LCP that are not supported by the data in a specific case; you don't have that here.   Writing a LCP based on "research" alone can be fatal. I think you have what you need from the doc if you can fine-tune his recommendations.  That will allow you to crunch the numbers you need.

    Bob I am not making medical rec's on treatment here and it's not my plan either. My question has to do with foundational literature that support an ortho treating a patient for their lifetime w PT and meds w lab work following two spinal surgeries. An Atty friend simply asked me if we have data on that.

    To all
    I agree with Mr. Taylor and his insights  and recommendations.
    I see plans where  the research ultimately contradicts the medical recommendations made by either the treating or examining physician.
