Anyone been asked to include end-of-life care, specifically hospice, to a Life Care Plan? The treating physician in this case insists his patients, due to their occupational disease, will enter hospice when their condition is deemed terminal and therefore, costs for hospice should be included in the LCP.  Research indicates a daily/per diem rate for hospice care which adds a significant cost estimate to the LCP ($228/day in this case) and is difficult to predict the length of need.

I have done so with cases where cancer was missed at the initial diagnosis.  Some venues I have found that there are foundations that provide this service as part of their mission and thus free whereas others there is the usual and customary cost.
