  • 1.  Life Care Planner in Wales/UK?

    I have a local law firm looking for a life care planner or equivalent in Wales/United Kingdom regarding a person who was originally injured in Ontario, Canada.  A quick search on IARP did not turn up any results.  If anyone knows of someone, please let me know and I will put them in touch.  Thanks in advance.

    I am in California however, I will be in England and Wales Sept and Oct partly work and partly vacation.  I am originally English, originally trained in UK, have worked in U of Western Ontario London, and been in US many years.   I would love to be able to help out on this.  What exactly is required?  I will be there from Sept 8 - Oct 28.  The first week is booked up but I will be available after that.

    I have done international work in Life Care Planning with a patient who sustained a severe traumatic brain injury in Vancouver, Washington USA but moved home to France. I traveled to France to assess her and meet with her physician team and family to complete comprehensive Life Care Plan based on care available to her in France.  I have also provided catastrophic case management for two American citizens injured abroad.  The first sustained a severe traumatic brain injury in Alberta Canada. I initiated case management at University of Edmonton in Alberta Canada Intensive Care Unit, flying back and forth to meet with his medical team until he was medically stable for transport. I then arranged for air ambulance transfer and direct admit to University of Utah Medical Center Intensive Care Unit.  My second patient was catastrophically injured in London, received initial care at Charing Cross Hospital intensive care unit (London) and air-transported and admitted directly to Emanuel Medical Center Burn Intensive Care Unit in (Portland Oregon).  In the course of establishing a case management plan I reviewed extensive acute care medical records from Charing Cross Hospital to assess his clinical status and delineate his initial care in order to put together a comprehensive rehabilitation team in America.   I am available to develop a Life Care Plan in Wales/United Kingdom.  Contact me at 503-701-9009 if I can be of assistance.
