    Hello All,

    Now that I am back at my desk at last, I am trying to decide which late report I should work on first...

    I wanted to ask about this one.  I was retained by the plaintiff atty. to do a Vocational and LCP assessment for a person injured in an MVA.  She has intermittent dizziness, balance issues, headaches, decreased stamina.  She reports feeling "foggy."  She reports increased symptoms with using the computer and must use the computer much of the time at work.

    She's had all the best treatment and seems to do a good job controlling symptoms as much as possible.  She works full-time in a relatively stressful & demanding administrative job at a major medical center, but notes her bosses are very understanding about her problems.  They tolerate her being late or even occasionally needing to lie down at work.  Coworkers have "covered" for her in terms of selected job duties.  The job she has is one in which she "worked her way up" over a period of 20-some years.  She would never get the same job today without advanced degrees. 

    Regarding home life, recreational activities are decreased.  She now goes to bed early due to fatigue from work. 

    There don't seem to be many future certain hard numbers attached to this one.  She likely needs help with outdoor work and heavy housekeeping. 

    Regarding vocational issues, she is earning a good income, so her greatest risk is related to if she would lose her job.  I can address that, but wonder if I am missing something in that regard.

    Much of this seems to me to be about her quality of life and pain & suffering.  I feel as if my conclusions so far are pretty vague.

    Any suggestions as to how I might do the best and most meaningful report in this situation?   

    Any thoughts appreciated!

    Hi Patty,
    I had a few thoughts when I read your post.
    I'm not sure what the injuries and resulting medical conditions are since her MVA, but if she is experiencing fatigue and strain from using her computer screen, she may be having neuro-ophthalmologic issues as a result of the trauma/MVA.  Has she been evaluated by a neuro-ophthalmologist?  They may be able to identify if there is some ocular nerve damage causing her symptoms and have some recommendations for future care/treatments to decrease strain and computer screen fatigue. 
    Also, has she been evaluated for mild TBI? If one hasn't been done already, maybe a neuropsych eval could help identify the reason for some of her symptoms.
    And maybe some testing of the inner ear may help identify if she has a disturbance or injury in her vestibular system causing the imbalance, dizziness and fogginess.
    Not knowing what has already been done for this evaluee, please take it all with a grain of salt!

    Patty, I wonder if this client has had a consult with an ENT and  an Audiologist post accident. Hearing  and vestibular fc'ns need to be addressed to pinpoint the cause(s) and provide her with appropriate treatment so that her QOL improves. Hope this helps.
