I am not sure how to handle this issue and wonder if anyone has any experience...I am doing two clinical interviews next week on two John Doe cases. (They are related). As is typical for the interview, I would request they each sign a medical authorization form. However, we are instructed to refer to them in any discoverable document as "John Doe" (although I do know each of their names)...
Do you think signing it by their given name for the case (JD - ###) is sufficient? Although I don't think I will need any additional medical records, I need authorization to speak to their providers, as I am doing a Life Care Plan.
