For the record, I am a duly Registered Speech-Language Pathologist in the Province of Ontario, a certified Speech-Language Pathologist and a Board Recognized Specialist in Child Language, Candidate with the American Speech-Language-Hearing Association, a registered Rehabilitation Professional with the Vocational Rehabilitation Association (formerly known as the Canadian Association of Rehabilitation Professionals - CARP), as well as, a certified Canadian Life Care Planner.
I am a clinical instructor for graduate students in Speech Pathology enrolled in Universities in Canada and USA, as well as a practicum supervisor for students training to be Communication Disorders Assistants in Ontario. I also mentor speech-language pathologists in accordance with the College’s (College of Audiologists and Speech-Language Pathologists of Ontario) requirements for mentorship of initial practice registrants. My clinical experience includes thirty one (31) years of practice as a Speech-Language Pathologist for clients requiring assessments and treatment of speech, voice, language, swallowing and cognitive-communication disorders. I also provide rehabilitation case management services to clients and their families since 1992. In addition to providing assessments, treatment, and rehabilitation services, I prepare life care plans for clients who acquired catastrophic challenges and difficulties as a result of an injury.
My referrals typically come from insurance companies, counsel for both defendant and plaintiff, physicians, specialists, audiologists, group homes, social workers, speech-language pathologists, hospital rehabilitation centers, the Workplace Safety and Insurance Board (formerly the Workers’ Compensation Board), rehabilitation case managers, nurses, and School Board personnel. Over the years, I have conducted numerous independent examinations (IMEs) and evaluations in multidisciplinary Designated Assessment Centres (DACs) of both children and adults. I have testified as an expert witness in Ontario Court on both Civil and Criminal matters. Between 1994 and 1998, I served on several committees representing the Ontario Association of Speech-Language Pathologists and Audiologists (OSLA) namely as a Chair on Auto Insurance. I was a member of the Protocol Committee and draft preparation for DACs, the provincial chair for paediatric DACs, and worked collaboratively with Ontario Insurance Commission & Health Professionals on revising SABS-Bill 59. In 1998, I completed a manual for CAT-DAC paediatrics together with other professionals, and submitted definitions for paediatrics on these committees. I was an early adopter of the Health Claims for Auto Insurance (HCAI) system and have contributed to the IE Guidelines (2010-2011) for Speech-Language Pathology on behalf of OSLA.
I am a member of the Canadian Society of Medical Evaluators (CSME), International Association of Rehabilitation Professionals (IARP), the Commission on Health Care Certification (CHCC), Vocational Rehabilitation Association (VRA), the International Academy of Life Care Planners (IALCP), Canadian Certified Life Care Planners (CCLCP), the Ontario Speech-Language Pathologists and Audiologists Association (OLSA), American Speech-Language-Hearing Association (ASHA), co-chair (1998) and past member (1999-2005) of the Neurorehabilitation Program Stakeholders’ Advisory Group Committee at Bloorview MacMillan Children’s Centre.
