Can anyone familiar with the Florida Workers' Comp Voc Rehab (Statute 440) please provide me with a sample report of format outline for a Reemployment interview/assessment ? I am a sole proprietor vocational case manager practicing in central PA. I have case where the injured worker is a relocated Florida resident, now living in south central PA
