  • 1.  Collaboration on Life Care Plans

    Hello Life Care Planners,

    I am curious how many life care planners with expertise in one area collaborate with other life care planners with expertise in another areas.  I have been collaborating with an occupational therapist who is a life care planner by providing physician recommendations.    I find this to be very beneficial as we bounce ideas off one another and learn from each other.

    Good Morning,
    I believe collaboration is important for our life care plans.  Our Standards of Practice state we need to collaborate for our plans.  Most of us are not experts in all areas.

    I agree Lisa.  Forecasting the needs of an individual decades into the future can be a daunting task. The two life care plans that I collaborated on were very strong and well thought out.  Both of us bought our unique perspectives to the table.

    I am an occupational therapist that works in the community specializing on home modifications and I rely on collaboration with other professionals in the community to provide comprehensive services for my client to remain in their homes. I learned how essential this was in the clinical/hospital setting and made sure I duplicated it when I went out on my own in the community. As a CLCP I value this same type of collaboration with other professionals, because I find it provides a thorough assessment of needs and a unique perspective that I, as an OT, would maybe not have seen.
