Raffle rules: You must be 18 or older to be eligible for the raffle. You do not have to participate in the survey to be eligible for participation in the raffle. Further, you will remain eligible for the raffle even if you choose to withdraw from the study or do not complete all questions. For those interested in participating in the raffle, we ask that you email the principal investigator to include your email address and desire to be entered into the raffle. All emails will be placed in a hat and 5 winners will be chosen by the principal investigator and student principal investigator. The winners will be notified via email on October 1, 2014. The winners will be asked to respond and confirm that they would like to receive the gift certificate. The gift certificate will then be emailed to the winner (20 dollar Amazon.com gift certificate). All other participants who provided their email address will be notified via email that they did not win the raffle. A record of the raffle will be maintained by the principal investigator and student principal investigator in an electronic file and will remain on a password protected computer. Any persons with an immediate relationship to the PI (spouse, parents, siblings, children) and any persons with a direct interest in the research study will be excluded from participation in the raffle.
