Job brief

CSG is seeking exceptional Independent Contractor Nurse Case Managers to enhance the quality of patient treatment plans , and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the patient’s recovery needs. Case management certification is a plus. We are looking for Nurse Case Managers in the state of New York and the state of Florida for field and telephonic case management for both auto and workers compensation cases.

Responsibilities

  • Handle case assignments, draft service plans, review case progress and determine case closure status.
  • Coordinate medically appropriate and necessary care that is safe, timely, effective, efficient, equitable, and patient-centered
  • Help clients achieve wellness and autonomy
  • Facilitate multiple care aspects (case coordination, information sharing, etc)
  • Develop effective working relationships and cooperate with medical team members throughout the entire case management process
  • Promote quality and cost-effective interventions and outcomes
  • Assess and address motivational and psychosocial issues
  • Adhere to professional standards as outlined by protocols, rules and regulations

Requirements

  • Graduate of a professional school of nursing
  • Excellent knowledge of case management principles, healthcare management and reimbursement structures
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Current license, certification or registration
  • BS degree in related healthcare field
